Report of Academic Dishonesty
 
Please forward all relevant materials to include highlighted or numbered similarities in documents, copies of suspected sources, the course syllabus, assignment instructions, SafeAssign reports, class postings, emails, and handouts within a day or two of this report. Please send documents to the Academic Judiciary email address at academic_judiciary@stonybrook.edu or send to Academic Judiciary, Melville Library E3310, Z+4=3351.

 

NOTE: Anonymous or incomplete forms will be discarded.

 

IN CASES OF MULTIPLE ACCUSATIONS: AN INDIVIDUAL REPORT MUST BE SUBMITTED FOR EACH STUDENT ACCUSED OF ACADEMIC DISHONESTY. ONLY ONE STUDENT PER REPORT.

 

INSTRUCTORS USING BLACKBOARD/SAFEASSIGN: Please be aware that the current semester's rosters (and SafeAssign reports) will be deleted the following semester. One month prior to deletion, Blackboard will notify all faculty via e-mail. If you have a pending Academic Dishonesty case at that time, please fill out the form linked in the e-mail to request your roster not be deleted.

 

SUBMITTING GRADES: IF YOU ARE SUBMITTING THIS REPORT AT THE END OF THE SEMESTER, PLEASE SUBMIT AN INCOMPLETE GRADE FOR THIS STUDENT ON THE GRADE ROSTER. PLEASE EMAIL THE FINAL COURSE GRADE WITH AND WITHOUT YOUR RECOMMENDED PENALTY. WE WILL REPORT THE FINAL GRADE TO THE REGISTRAR ONCE THE MATTER IS RESOLVED.
