Excellence in Clinical Practice in the Community Award

Excellence in Clinical Practice in the Community Award (surgical/medical specialty). Awarded to the faculty member (surgical/medical specialty) who best exemplifies a devotion to delivering quality care and teaching in the Community. Nominee considerations include the candidate's contribution to their community, their professional reputation, and their noteworthy achievements. The strength of the nomination letter and letters of recommendation will also be taken into consideration.

Submission Guidelines
Each of the awards requires a nomination letter and three to five letters of support (preferably including a letter of support by the department chair) along with an abbreviated curriculum vitae and other documentation to support the award.

All submissions must be completed electronically, preferably as a SINGLE PDF file. The file name should be the first initial last name of the nominee and the date of submissions (eg jdoe061708). If there are more than one set of files, please label them as above with an additional digit (eg jdoe061708-1, jdoe061708-2, etc). No packet shall be more than 20 pages including appendices.

Nominations will be accepted up to July 31, 2024. Awards will be presented at our Annual Faculty Awards Ceremony in Fall 2024.
