• You will follow all campus guidelines related to COVID-19, including isolation and quarantine if required.
• The information you submit, including your test result, is truthful and accurate.

Lastly, if it is discovered that you are untruthful in your submission, you will be referred to the Academic Judiciary Office for investigation of a violation of academic dishonesty. If found responsible for academic dishonesty, you will be subject to sanctions that may include suspension or expulsion from the university.
